pemetrexed

Overview

Pemetrexed is a multitargeted antifolate that inhibits thymidylate synthase, dihydrofolate reductase, and glycinamide ribonucleotide formyltransferase, disrupting nucleotide synthesis. Combined with platinum (cisplatin or carboplatin), it is a standard first-line regimen for non-squamous NSCLC and malignant pleural mesothelioma.

Evidence in the corpus

  • Antifolate chemotherapy; contraindicated throughout pregnancy (teratogenic) in young-onset NSCLC patients despite being a standard first-line NSCLC agent; review recommends platinum-taxane or platinum-vinca combinations as alternatives during pregnancy PMID:27346245
